Job Summary
To enhance patient care coordination, the full-time Oregon and Washington Licensed Care Coordinator will manage a moderate caseload by reviewing treatment plans, collaborating with clinical teams, and ensuring compliance with health regulations while working remotely.
Key Responsibilities
- Coordinate patient care by collaborating with clinical teams to review and improve treatment plans and ensure smooth transitions across the continuum of care
- Manage patient data and records, compiling and reporting information to identify opportunities for interventions and conducting quality management studies
- Ensure compliance with policies and procedures while advocating for patients and facilitating relationships with external stakeholders
Required Qualifications
- Minimum two (2) years of experience in behavioral health case management or care coordination
- Master's degree in Psychology, Counseling, Social Work, or a related field with a minimum of three (3) years of experience in a related field
- Licenses required within six months of hire: Licensed Clinical Social Worker in Oregon and Licensed Independent Clinical Social Worker in Washington, or equivalent licenses
- National Provider Identifier required at hire
