Oregon Care Coordinator

Job Summary

Providing field care coordination outreach activities, the full-time Oregon Care Coordinator will support care management program delivery to plan enrollees, ensuring comprehensive services and conducting assessments, all while working remotely within the state.

Key responsibilities

• Manage inbound and outbound calls to support enrollees effectively
• Conduct assessments for case manager or health coach assignment based on evaluation results
• Participate in interdisciplinary case reviews to ensure quality care and effective coordination planning

Required qualifications

• Associate's degree in a health-related field or medical assistant training
• 4+ years of experience in care coordination, preferably in a field environment
• 2+ years of clinical experience, ideally in field delivery
