Job Summary

A company is looking for an Accountability Coach (Contracted).

Key Responsibilities

• Serve as an expert diabetes health coach to support individuals in achieving optimal health outcomes
• Engage members empathetically through telephone calls and text messaging
• Deliver education and personalized guidance according to protocols to aid members in self-managing their condition

Qualifications

• National Board-Certified Health & Wellness Coach (NBC-HWC) or LPN or higher clinical licensure required
• Experience in chronic disease management, particularly diabetes, is preferred
• Willingness to learn and become proficient with necessary technology
• Experience or training in Motivational Interviewing is beneficial
• Bilingual (Spanish) is a plus
