Job Summary

A company is looking for an Advanced Care at Home RN.

Key Responsibilities

• Provide high-quality patient care to acute care populations in the home setting
• Monitor and respond to biometric data and conduct home visits as directed
• Implement patient care plans efficiently while adhering to clinical protocols

Required Qualifications

• Active, unencumbered Oregon state nursing license
• RN Compact (multi-state) license preferred
• Three years of RN experience, with Med/Surg or inpatient nursing experience required
• Clinical knowledge and commitment to patient safety
• Willingness to engage in ongoing learning and process improvement efforts
