Job Summary

To support a growing team, the full-time Oregon Licensed Behavioral Health Case Manager will evaluate beneficiaries' needs, develop individualized care plans, and coordinate with healthcare team members to ensure optimal client outcomes while working remotely within the state of Oregon.

Key responsibilities
  • Assess, plan, implement, monitor, and evaluate individualized care plans for beneficiaries across the continuum of care
  • Collaborate with beneficiaries, providers, and healthcare team members to coordinate care and address gaps in compliance
  • Maintain accurate documentation and ensure adherence to confidentiality and regulatory standards
Required qualifications
  • Current, unrestricted license or certification to practice a health or human services discipline in Oregon
  • Bachelor's degree in Social Work, Public Health, Human Services, Healthcare Administration, or a related field
  • 2+ years of experience in Medicaid funded case management, care coordination, or community health programs
  • Experience working with high-need populations, including I/DD, aging adults, and SMI
  • Demonstrated ability to build strong partnerships with community mental health providers and organizations
