Job Summary
Seeking an Oregon Licensed Behavioral Health Case Manager, the full-time remote position will evaluate beneficiaries' needs, develop individualized care plans, and coordinate with healthcare team members to ensure optimal client outcomes while adhering to payer requirements.
Key responsibilities
- Assess, plan, implement, monitor, and evaluate individualized care plans for beneficiaries
- Develop beneficiary-centered care plans and facilitate integrated case management activities
- Maintain compliance with organizational, state, and federal regulations regarding client confidentiality and documentation
Required qualifications
- Bachelor's degree in Social Work, Public Health, Human Services, Healthcare Administration, or a related field
- Current, unrestricted Oregon license or certification to practice a health or human services discipline
- 3+ years of Social Work experience with the older adult population
- 3+ years of clinical experience
- 1+ years of case management review experience
