Job Summary
To support integrated care within the Family Medicine clinic, the full-time Oregon Licensed Behavioral Health Consultant will provide primary care behavioral health interventions, engage in case management, and teach clinic learners while working onsite with some remote opportunities.
Key responsibilities
- Provide primary care behavioral health interventions including assessments, crisis intervention, and follow-ups
- Participate in multi-disciplinary case management to address complex patient needs
- Teach clinic learners about behavioral health issues and evidence-based interventions
Required qualifications
- Master's Social Work (MSW) required; health care experience preferred
- Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW) required
- Training in Primary Care Behavioral Health preferred
- Experience as a Behavioral Health Consultant in a Primary Care setting preferred
- Understanding of patient-centered medical home principles
