Job Summary
To support integrated care within the Family Medicine clinic, the full-time Oregon Licensed Behavioral Health Consultant will provide evidence-based interventions, participate in multi-disciplinary case management, and engage in teaching while maintaining a professional presence in a clinical setting.
Key responsibilities
- Deliver primary care behavioral health interventions, including assessments and crisis intervention, while documenting services in the EHR
- Collaborate with the care team to manage complex patient needs and ensure continuity of care
- Teach clinic learners about behavioral health issues and evidence-based interventions, enhancing patient engagement strategies
Required qualifications
- Master's Social Work (MSW) required; health care experience preferred
- Certificate of Clinical Social Work Associate (CSWA) or Licensed Clinical Social Worker (LCSW) required
- Training in Primary Care Behavioral Health is preferred
- Experience as a Behavioral Health Consultant in a Primary Care setting is advantageous
- Demonstrated commitment to diversity, equity, and inclusion in clinical practice
