Oregon Licensed Behavioral Health Navigator

Job Summary

A company is looking for a Behavioral Health Navigator (QMHA) to coordinate care for behavioral health needs remotely in Oregon.

Key Responsibilities

• Coordinate care between members, primary care, emergency departments, and behavioral health providers
• Connect members to community resources, including housing and employment services
• Monitor and document progress of care coordination plans and assist with care management duties

Required Qualifications

• Two years of experience in healthcare administration or case management
• Strong knowledge of community resources and behavioral health providers
• High School diploma or equivalent
• Qualified Mental Health Associate (QMHA) certification or ability to obtain
• Bilingual in Spanish preferred
