Job Summary

A company is looking for a Care Coordinator - Healthcare (Remote within Oregon and bordering geographies).

Key Responsibilities

• Provide telephonic care coordination outreach to support preventative health activities and reduce barriers to accessing care
• Conduct health risk assessments and maintain metrics for telephonic outreach and member engagement
• Participate in team case reviews for collaborative assessment and coordination planning

Required Qualifications

• A minimum of a High School Diploma or GED equivalent
• 2+ years of experience working in a healthcare environment
• 1+ years of care coordination experience
• Knowledge of medical terminology
