Job Summary

Oregon Licensed Care Coordinator is a full-time position responsible for telephone outreach to member populations to ensure service utilization and satisfaction while meeting compliance regulations.

Key Responsibilities

• Conduct outreach to members regarding emergency department usage and missed dental appointments
• Assist in care coordination and reassignment of members needing dental services
• Ensure accurate and timely documentation in case management systems for continuity of care

Required Qualifications

• High school diploma or equivalent
• 6+ months of experience working with Medicaid populations
• Computer proficiency in various Windows applications
• Willingness to attend additional training as needed
