Oregon Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Develops and facilitates complex care management activities, including personalized care plans for members
• Coordinates care between members, families, and providers to ensure appropriate person-centered services
• Monitors member progress and revises care plans as necessary to meet goals and address unmet needs

Required Qualifications, Training, and Education

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Preferred: Residency in Lane County, Oregon
• Preferred: Certified Case Manager (CCM) credential and bilingual proficiency in an additional language
