Job Summary

A company is looking for a Care Manager LPN LVN for a remote position in case and disease management.

Key Responsibilities

• Provide care coordination services, including disease management and case management for members with complex health needs
• Educate and empower members to manage their diseases effectively
• Assist with transition of care planning and end-of-life care planning

Required Qualifications

• Certificate/Diploma from a state-approved practical nursing program
• Associate's Degree in Healthcare or a related field
• Oregon Licensed Practical Nurse upon hire
• 5 years of clinical nursing experience
• 2 years of experience collaborating with physicians in patient care management
