Job Summary

A company is looking for a Mid Case Manager.

Key Responsibilities

• Conduct outreach, engagement, and follow-up with referred members
• Develop, update, and monitor Person Centered Service Plans (PCSPs)
• Coordinate across medical, behavioral health, and social service systems to reduce barriers to care

Required Qualifications

• Bachelor's degree in Social Work, Public Health, Human Services, or a related field
• 2+ years of experience in Medicaid funded case management or care coordination
• Experience working with high need populations
• Ability to build partnerships with healthcare providers and community organizations
• Experience in providing technical assistance and best practice implementation support
