Job Summary
To support individuals with significant behavioral health needs, the full-time Oregon Licensed Case Manager will conduct outreach, develop service plans, and coordinate care remotely while ensuring compliance with OHA and CMS requirements.
Key responsibilities
- Conducting timely outreach, engagement, and follow-up with referred members
- Developing, updating, and monitoring Person Centered Service Plans (PCSPs)
- Coordinating across medical, behavioral health, and social service systems to reduce barriers to care
Required qualifications
- High school diploma or equivalent
- 2+ years of experience in Medicaid funded case management or care coordination
- Experience working with high-need populations such as aging adults or individuals with disabilities
- Demonstrated ability to build strong partnerships with healthcare providers and community organizations
- Experience in providing technical assistance and best practice implementation support
