Job Summary

To support individuals with significant behavioral health needs, the full-time Oregon Licensed Case Manager will conduct outreach, develop service plans, and coordinate care remotely while ensuring compliance with OHA and CMS requirements.

Key responsibilities
  • Conducting timely outreach, engagement, and follow-up with referred members
  • Developing, updating, and monitoring Person Centered Service Plans (PCSPs)
  • Coordinating across medical, behavioral health, and social service systems to reduce barriers to care
Required qualifications
  • High school diploma or equivalent
  • 2+ years of experience in Medicaid funded case management or care coordination
  • Experience working with high-need populations such as aging adults or individuals with disabilities
  • Demonstrated ability to build strong partnerships with healthcare providers and community organizations
  • Experience in providing technical assistance and best practice implementation support
