Oregon Licensed Case Manager RN

Job Summary

To enhance care delivery and outcomes, the full-time Oregon Licensed Case Manager RN will provide care coordination, manage beneficiary needs across various health services, and collaborate with a multidisciplinary team in a remote setting within Oregon.

Key responsibilities:

• Provide care coordination and management for beneficiaries, ensuring high clinical performance by the team
• Educate beneficiaries about community resources and advocate on their behalf
• Complete health and social assessments to develop personalized care plans using motivational interviewing and trauma-informed care principles

Required qualifications:

• Current, active, and unrestricted Oregon Registered Nursing license
• Associate's degree in nursing or bachelor's degree from an accredited college or university
• 2+ years of clinical experience
• 1+ years of case management or home care experience
• Must reside in Oregon
