Job Summary

To enhance care delivery, the full-time Oregon Licensed Case Manager RN will manage personalized care planning and coordination for beneficiaries, ensuring compliance with payer standards while working remotely.

Key responsibilities

• Provide care coordination and management for beneficiaries, ensuring high clinical performance by the team
• Collaborate with a multidisciplinary care team to assess and address beneficiaries' diverse health needs
• Complete health and social assessments for care planning, utilizing motivational interviewing and trauma-informed care principles

Required qualifications

• Current, active, and unrestricted Oregon Registered Nursing license
• Associate's degree in nursing or bachelor's degree from an accredited college or university
• 2+ years of clinical experience
• 1+ years of case management or home care experience
