Oregon Licensed Clinical Care Coordinator

Job Summary

A company is looking for a Behavioral Health Care Management Clinician (remote in Oregon).

Key Responsibilities

• Document and present clinical assessments and case formulations
• Develop individualized plans of care and provide telephonic support for members
• Coordinate with various healthcare providers to ensure access to appropriate services

Required Qualifications

• 5 years of experience in mental health and knowledge of DSM and ASAM criteria
• Experience with programs such as EASA, IIBHT, or coordinating care between levels
• Master's degree in social work, psychology, sociology, or a related field
• Licensed Clinical Social Worker or other Oregon licensed healthcare profession required
• Bilingual in Spanish preferred
