Job Summary

A company is looking for a Clinical Pharmacist Digital Rx.

Key Responsibilities

• Design, develop, and implement population-level initiatives and medication management strategies
• Deliver high-quality clinical services and comprehensive medication reviews to support patients and care teams
• Provide Subject Matter Expertise for projects and contribute to the design and improvement of clinical solutions

Required Qualifications

• Bachelor's degree in Pharmacy or a PharmD or related field
• 3-5 years of experience in ambulatory or acute care or equivalent combination of education and experience
• An active and unrestricted pharmacy license in state(s) of practice
