Oregon Licensed Clinical Pharmacist

Job Summary

A company is looking for a Clinical Pharmacist to provide integrated support to patients and physicians to improve the quality of patient care.

Key Responsibilities

• Review and interpret clinician's patient care notes for prior authorization approvals and care plan recommendations
• Monitor ongoing patient adherence and evaluate therapies for side effects, recommending changes as necessary
• Supervise support personnel and maintain compliance with local, state, and federal drug laws

Required Qualifications

• Doctor of Pharmacy (PharmD) or equivalent advanced degree
• Licensed Pharmacist in the state of Oregon
• 1+ years of experience preferred but not required
• Knowledge of complex/chronic disease states and therapies used within specialty pharmacy
• Commitment to obtaining any additional necessary licensure within an agreed timeframe
