Job Summary

A company is looking for a Clinical Pharmacy Specialist who will provide clinical pharmacy services to members of a health plan.

Key Responsibilities

• Provide clinical pharmacy services and medication management to members
• Document clinical case reviews and consult on medication information with healthcare providers
• Develop and review clinical policies and medication evidence for population health management

Required Qualifications

• Pharm D or Bachelor's/Master's Degree in Pharmacy with 5 years of managed care experience
• PGY-1 Residency training with relevant practice experience or PGY-1 Managed Care Residency
• Oregon or Washington State Pharmacist License
• National Certification from Board of Pharmacy Specialties within 2 years of hire
• Experience in writing medical papers and educational materials for patients and providers
