Job Summary

To elevate patient care at a nationally recognized Academic Medical Center, the remote Oregon Licensed Clinical Pharmacy Specialist will provide comprehensive medication management, collaborate with care teams under a Collaborative Practice Agreement, and optimize therapy for patients.

Key responsibilities
  • Provide comprehensive medication management, including reconciliation, dose optimization, and adherence support
  • Partner with physicians and interdisciplinary teams to order individualized care plans and ensure seamless medication coordination
  • Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as necessary
Required qualifications
  • Doctor of Pharmacy (PharmD) degree
  • Licensed pharmacist in Oregon
  • Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency with at least 1 year of ambulatory practice under a CPA, or at least 3 years of ambulatory practice with disease state management
  • Strong communication, organization, and technology skills
  • Empathy, integrity, and a team-first mindset
