Job Summary
To elevate patient care at a nationally recognized Academic Medical Center, the part-time Oregon Licensed Clinical Pharmacy Specialist will provide comprehensive medication management, collaborate with care teams under a Collaborative Practice Agreement, and conduct patient assessments remotely.
Key responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, and adherence support
- Collaborate with physicians and interdisciplinary teams to order individualized care plans and ensure seamless medication coordination
- Monitor therapy effectiveness and safety, identifying optimization opportunities and intervening as necessary
Required qualifications
- Doctor of Pharmacy (PharmD) degree
- Licensed pharmacist in Oregon
- Certified as an HIV Specialist (AAHVIP)
- Completion of a PGY2 Ambulatory Care Residency or PGY1 Residency with at least 1 year of ambulatory practice under a CPA, or at least 3 years of ambulatory practice with disease state management
- Strong organizational and technical skills
