Job Summary

A company is looking for a Clinical Program Coordinator RN - Remote.

Key Responsibilities

• Provide care coordination services including disease management and case management
• Assist members with managing health plan benefits and accessing alternative services
• Work to improve access and quality of care for members with complex health needs

Required Qualifications

• 5 years of clinical nursing experience
• Current unencumbered Registered Nurse License in state of residency upon hire
• Experience collaborating with physicians in patient care management
