Job Summary

A company is looking for a Clinical Program Coordinator RN - Case and Disease Program - Remote.

Key Responsibilities

• Provide care coordination services to members with acute and complex health care needs
• Manage disease management programs and case management including triage, referral, and transition of care planning
• Educate and empower members to effectively manage their diseases and health outcomes

Required Qualifications

• Current unencumbered Registered Nurse License in state of residency upon hire
• 5 years of clinical nursing experience
• Experience collaborating with physicians in patient care management
