Job Summary

A company is looking for an LVN Case Manager - Disease Management - Remote.

Key Responsibilities

• Conduct clinical evaluations and assessments to determine case management eligibility and individual needs
• Develop and implement case management plans in collaboration with members and healthcare professionals
• Maintain a member caseload for regular outreach, support, and documentation of interventions and goal achievements

Required Qualifications

• Graduation from an accredited Licensed Vocational Nurse program
• Active compact nursing license in WA, ID, UT, CO, NM, AZ, or OR
• 2+ years of recent clinical experience as an LVN/LPN
• 1+ years of care management, disease management, or discharge planning experience
• Access to a dedicated workspace from home with high-speed internet connection
