Job Summary
To support a growing Senior Community Care program, the full-time Oregon Licensed Nurse Practitioner will provide virtual care and manage high-risk patients in various settings, collaborating with an interdisciplinary team while working remotely from home.
Key responsibilities
- Manage care coordination for members via telephonic and video platforms, ensuring effective communication among caregivers and healthcare providers
- Conduct routine and comprehensive assessments, managing acute medical situations and chronic condition exacerbations
- Document medical information accurately and engage in advanced care planning conversations with patients and caregivers
Required qualifications
- Active and unrestricted Oregon Nurse Practitioner license and ability to obtain licenses in other required locations
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
- 3+ years of clinical experience in practice
- Current, active DEA licensure or prescriptive authority, or ability to obtain post-hire
