Job Summary
To support a growing Senior Community Care program, the full-time Oregon Licensed Nurse Practitioner will provide virtual care for high-risk health plan members, coordinating and managing care while working remotely from a secure location.
Key responsibilities:
- Manage care for members through telephonic and video platforms, coordinating with caregivers and healthcare providers
- Conduct routine, acute, and comprehensive assessments, managing members during acute medical situations and rehabilitation episodes
- Document patient information accurately and collaborate with the interdisciplinary care team to provide comprehensive care
Required qualifications:
- Active and unrestricted Nurse Practitioner license in Oregon and the state of residence
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Board certified as a Nurse Practitioner by a recognized organization
- 3+ years of clinical experience in practice
- Current DEA licensure/prescriptive authority or ability to obtain post-hire
