Job Summary
To support a growing practice, the full-time Oregon Licensed Physical Therapist Care Navigator will guide patients through their musculoskeletal care journey by providing clinical expertise, triaging care pathways, and ensuring effective communication between patients and healthcare providers in a remote setting.
Key responsibilities
- Guide clinical decision-making by conducting assessments and resolving complex patient situations
- Build trusted relationships with patients through high-touch telephonic engagement and proactive follow-up
- Coordinate care across the healthcare ecosystem, ensuring collaboration among patients, caregivers, and providers
Required qualifications
- Doctor of Physical Therapy (DPT), Doctor of Physiotherapy, or Master of Physical Therapy (MPT)
- Active, unrestricted Physical Therapist license in Oregon
- 5+ years of orthopedic or musculoskeletal clinical experience
- Strong clinical judgment and triage skills
- Experience in motivational interviewing and patient coaching
