Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with Oregon Licensure for a remote position.

Key Responsibilities

• Manage patient census and address gaps in clinical and non-clinical care
• Identify and coordinate community resources for chronic care management
• Provide patient education on managing chronic conditions and assist with medication management

Required Qualifications

• Active Oregon License as a Licensed Practical Nurse (LPN)
• Minimum of 3 years of direct patient-facing experience
• Technical proficiency in Microsoft Office Suite
• Ability to work in a sedentary position for extended periods
• Must have a home office or HIPAA-compliant workspace with high-speed Wi-Fi
