Oregon Licensed Practical Nurse

Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with Oregon Licensure for a remote, day shift position.

Key Responsibilities

• Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources that would benefit patient care
• Provide patient education on managing chronic conditions and perform medication management

Required Qualifications

• Active Oregon License (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a remote, HIPAA-compliant environment
• Experience with Epic is a plus
