Oregon Licensed Primary Care NP/PA

Job Summary

To provide innovative virtual primary care, the full-time Oregon Licensed Primary Care NP/PA will manage a panel of patients, conducting comprehensive assessments and developing chronic condition management plans through a telehealth platform.

Key responsibilities

• Deliver longitudinal virtual primary care visits via telehealth to a panel of patients
• Diagnose and manage chronic conditions, developing individualized care plans to optimize patient health
• Collaborate with a multidisciplinary team to coordinate in-person services and facilitate referrals as needed

Required qualifications

• Active Advanced Practice Provider (APRN or PA) license in good standing in Oregon
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
• Minimum 4 years of primary care, urgent care, or internal medicine experience, preferably in telehealth settings
• Experience managing chronic conditions and preventive care via telehealth
• Active BLS certification at the time of hire
