Job Summary

Seeking a compassionate and skilled Oregon Licensed Primary Care Provider, the full-time Advanced Practice Provider (NP/PA) will deliver virtual primary care through a telehealth platform, managing chronic conditions, preventive screenings, and acute care visits while collaborating with a multidisciplinary team.

Key responsibilities:

• Provide virtual primary care visits, including assessments, chronic disease management, and acute care interventions
• Diagnose and manage chronic conditions, developing individualized care plans to optimize patient health
• Order and interpret lab tests and diagnostics, coordinating follow-up care as necessary

Required qualifications:

• Active Nurse Practitioner (APP) or Physician Assistant license in good standing
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine
• Minimum 2 years of primary care, urgent care, or internal medicine experience, preferably in telehealth
• Active BLS certification at time of hire
• Ability to obtain licensure in multiple states within 90 days of hire
