Oregon Licensed RN Care Coordinator

Job Summary

To ensure timely and appropriate care delivery, the full-time Oregon Licensed RN Care Coordinator will perform care coordination duties, interact with members and providers to develop treatment plans, and utilize internal systems for eligibility and benefit determinations while working remotely.

Key responsibilities

• Investigates healthcare alternatives and prepares cost-effective plans for patients and providers
• Coordinates with health services vendors and community resources to support member care
• Maintains an acceptable caseload and ensures compliance with medical policies and standards of care

Required qualifications

• Graduate of an accredited 2-4 year nursing program or a Master's Degree in Social Work
• Current unrestricted Oregon Registered Nurse license
• Three years of recent experience in hospital, home health, or acute care settings
• Proficiency in Microsoft Office applications and typing speed of at least 35 wpm
• Strong analytical and organizational skills with the ability to manage shifting priorities
