Oregon Licensed RN Care Coordinator

Job Summary

To ensure timely and appropriate care delivery, the full-time Oregon Licensed RN Care Coordinator will perform care coordination, interact with members and providers to develop treatment plans, and utilize internal systems for eligibility and benefit determinations while working remotely.

Key responsibilities

• Investigate healthcare alternatives and prepare cost-effective care plans for patients and providers
• Coordinate with health services vendors and community resources to support member care
• Maintain an acceptable caseload while ensuring compliance with medical policies and performing Utilization Reviews

Required qualifications

• Graduate of an accredited 2-4 year nursing program
• Current unrestricted Oregon Registered Nurse license
• Three years of recent experience in hospital, home health, or acute care settings
• Proficiency in Microsoft Office applications and ability to type a minimum of 35 wpm
• Strong analytical and organizational skills with the ability to manage shifting priorities under pressure
