Oregon Licensed RN Case Manager

Job Summary

A company is looking for an RN Case Manager to perform case management for members, ensuring timely and appropriate care delivery.

Key Responsibilities

• Conduct assessments, planning, implementation, coordination, monitoring, and evaluation of case management activities
• Collaborate with care providers and consult with physician advisors to ensure compliance with medical policies and standards of care
• Document interactions and maintain communication with members, providers, and regulatory organizations while protecting confidentiality

Required Qualifications, Training, and Education

• Graduate of an accredited nursing program (two, three, or four-year)
• Current unrestricted Oregon Nursing license as a registered nurse
• Certification in case management (CCMC) required or must be obtained within 24 months of hire
• Minimum of 3 years recent experience in hospital, home health, or acute care settings
• Proficient in Microsoft Office applications and typing a minimum of 35 wpm
