Oregon Licensed RN Case Manager

Job Summary

To ensure timely and appropriate care delivery for members, the full-time Oregon Licensed RN Case Manager will perform case management activities including assessment, planning, implementation, coordination, and monitoring of treatment plans while working from home.

Key responsibilities

• Conduct comprehensive assessments to identify individual member needs and develop tailored treatment plans
• Coordinate and implement case management activities to achieve established goals and objectives
• Monitor treatment effectiveness and collaborate with healthcare providers to resolve care-related issues

Required qualifications

• Graduate of an accredited nursing program with an unrestricted Oregon RN license
• Certification in case management (CCMC) required or ability to obtain within 24 months of hire
• Minimum of 3 years recent experience in hospital, home health, or acute care settings
• Proficiency in Microsoft Office applications and typing speed of at least 35 wpm
• Demonstrated ability to manage complex situations and communicate effectively with diverse stakeholders
