Oregon Licensed RN Case Manager

Job Summary

To ensure timely and appropriate care for members, the full-time Oregon Licensed RN Case Manager will perform case management activities including assessment, planning, implementation, coordination, and monitoring of treatment plans while working from home.

Key responsibilities

• Conduct comprehensive assessments to identify individual member needs and develop tailored treatment plans
• Coordinate and implement case management activities to achieve set objectives and goals
• Monitor and evaluate the effectiveness of treatment plans, ensuring compliance with medical policies and standards of care

Required qualifications

• Graduate of an accredited nursing program with a current unrestricted Oregon Nursing license
• Certification in case management (CCMC) required or ability to obtain within 24 months of hire
• Minimum of 3 years of recent hospital, home health, or acute care experience
• Proficiency in Microsoft Office applications and typing at least 35 wpm
• Strong analytical skills and ability to manage challenging customer interactions effectively
