Oregon Licensed RN Case Manager

Job Summary

To ensure timely and appropriate care delivery, the full-time Oregon Licensed RN Case Manager will perform case management activities, including developing and monitoring treatment plans, while working remotely.

Key responsibilities

• Conduct comprehensive biopsychosocial assessments to identify member needs and develop individualized care plans
• Coordinate services across behavioral health, medical, and community systems to promote integrated care
• Engage members and families using motivational interviewing and trauma-informed communication to support treatment participation

Required qualifications

• Current unrestricted Oregon license as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW)
• Graduate of an accredited school of Nursing or Social Work
• Certification in case management (CCMC) required or ability to obtain within 24 months of hire
• Recent healthcare experience in acute care and/or skilled nursing care, including medical, surgical, and behavioral health knowledge
• Proficient with Microsoft Office applications and ability to type a minimum of 35 wpm
