Job Summary

Key Responsibilities

Develops and assesses ongoing care plans and collaborates with providers to address member needs
Coordinates care between members, families, and care providers to ensure appropriate services
Monitors and updates care plans based on member status and collaborates with stakeholders to meet goals

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
4 - 6 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure required
Must have an Oregon RN license
