Job Summary
To support a growing telehealth practice, the full-time Oregon Licensed Telehealth Nurse Practitioner will manage high-risk patients remotely, providing clinical assessments, annual wellness exams, and coordinating continuity of care to optimize patient health outcomes.
Key responsibilities:
- Assess patients' healthcare needs and provide clinical care and consultation via telemedicine
- Perform annual wellness exams for senior Medicare patients and coordinate continuity of care with primary healthcare providers
- Document patient visits and maintain medical records in compliance with HIPAA regulations
Required qualifications:
- Current Oregon Nurse Practitioner license
- 2+ years of clinical experience as a Nurse Practitioner in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care
- Nurse Practitioner certification from either the ANCC or AANP
- Current DEA licensure and prescriptive authority
- Active CPR/BLS Certification
