Job Summary
To support a growing telehealth practice, the part-time Oregon Licensed Telehealth Nurse Practitioner will manage high-risk patients remotely, providing clinical assessments, annual wellness exams, and coordinating care to prevent unnecessary acute admissions, working 4-6 days per month.
Key responsibilities
- Assess patients' healthcare needs and provide clinical care and consultation via telemedicine
- Perform annual wellness exams and coordinate continuity of care under the direction of primary healthcare providers
- Collaborate with an interdisciplinary team to ensure holistic patient care and complete documentation within 72 hours of visits
Required qualifications
- Current Oregon Nurse Practitioner license
- 2+ years of clinical experience as a Nurse Practitioner in relevant fields such as Primary Care or Gerontology
- Nurse Practitioner certification from either the ANCC or AANP
- Current DEA licensure / Prescriptive Authority required
- Active CPR/BLS Certification
