Oregon RN Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop ongoing care plans and collaborate with providers to address member needs
• Provide ongoing follow-up and monitor member status, revising care plans as necessary

Required Qualifications, Training, and Education

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Must hold an Oregon RN license and be willing to obtain a Washington RN license
