Oregon RN Care Navigator

Job Summary

To support kidney health management, the 100% remote Oregon RN Care Navigator will manage a caseload of members, facilitating high-quality care coordination, conducting health screenings, and collaborating with healthcare professionals to optimize patient outcomes.

Key responsibilities

• Handle in and outbound calls to deliver exceptional service and educate members on kidney health and treatment options
• Engage members in HealthMap's Kidney Health Program while following up based on complexity and policy cadence
• Act as a liaison among patients, their support networks, and healthcare providers to coordinate care and resolve barriers

Required qualifications

• Active, unrestricted RN license required
• Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis center or transplant center preferred
