Job Summary
To support members in achieving their health goals, the fully remote OTA Licensed Care Specialist will develop personalized care plans, provide health coaching, and assist with care coordination while fostering supportive relationships with members and their caregivers.
Key responsibilities
- Develop trusting relationships with members to facilitate behavior change and health management through motivational interviewing techniques
- Coordinate members' relationships with various service systems, providing educational resources and recommendations for care adaptations
- Encourage adherence to medical treatment plans and promote the adoption of healthier habits for chronic condition management
Required qualifications
- College degree in a health-related field preferred
- Current and unrestricted OTA license/certification may be required
- Minimum 1-3 years of clinical experience with patient assessments
- Experience working with the Medicare population
- Familiarity with Motivational Interviewing techniques and behavioral change theory
