OTA or PTA Licensed Care Specialist

Job Summary

A company is looking for a Care Specialist to support members in improving their health through a Whole Person Care approach.

Key Responsibilities

• Develop trusting relationships with members to facilitate behavior change and health management
• Assist in coordinating care and provide educational resources tailored to members' needs
• Create individualized health plans and serve as a resource for Care Management and team members

Required Qualifications

• College degree in a health-related field preferred
• Current and unrestricted OTA or PTA license/certification required
• Minimum 1-3 years of clinical experience with patient assessments
• Experience with Motivational Interviewing techniques and working with the Medicare population
• Proficient computer skills and ability to work remotely
