Job Summary
A company is looking for an Outreach Coordinator to enhance member health outcomes through community collaboration and member engagement.
Key Responsibilities
- Identify and collaborate with community organizations to improve member health outcomes
- Make outbound calls to assist members in obtaining services and improving health outcomes
- Develop educational materials and maintain member incentive programs to support health initiatives
Required Qualifications
- 3+ years of recent outreach experience with providers or members, or government program managed care experience
- Experience with Microsoft Office products (Word, Excel, Project, PowerPoint, Outlook)
- High School Diploma or GED (required)
