Outreach Specialist

Job Summary

To support the growth of a geriatric care model, the full-time Outreach Specialist will engage with prospective and existing patients through inbound and outbound calls, schedule in-home wellness visits, and assist with appointment confirmations and rescheduling in a remote work environment.

Key responsibilities

• Conduct outbound calls and receive inbound calls to schedule and confirm in-home wellness visits for patients
• Provide information to potential new patients about WellBe's services and assist them in navigating their healthcare needs
• Support the in-home care team by delivering necessary information to engage patients in their health journey

Required qualifications

• High School diploma or GED equivalent; some college or Associate's Degree preferred
• At least three years of outbound call center and/or customer service experience, preferably in healthcare
• Strong communication skills with the ability to convey information clearly and effectively
• Proficiency in Microsoft Office Suite and call center software applications
• Ability to work independently and make sound decisions while adhering to company guidelines
