OVR Transition Coordinator

Job Summary

A company is looking for a Transition Coordinator to support students and participants with disabilities in preparing for employment and independence.

Key Responsibilities

• Work with adolescents aged 14-21 to develop and implement transition programming
• Coordinate career exploration, job readiness, and community-based experiences
• Serve as a liaison between programs and external partners while maintaining documentation and compliance

Required Qualifications, Training, and Education

• Must be based in the Hampden, PA area
• Experience working with individuals with disabilities
• Knowledge of vocational rehabilitation processes
• Ability to collaborate with educators and community agencies
• Relevant educational background in social work, education, or a related field
