PA and NJ Licensed Clinical Care Manager

Job Summary

To support high-risk patient populations, the full-time PA and NJ Licensed Clinical Care Manager will manage care plans, collaborate with healthcare teams, and provide clinical support through telehealth and in-person encounters while working remotely with occasional meetings in Bala Cynwyd, PA.

Key responsibilities

• Designs and implements care management activities for patients with chronic diseases and high medical service utilization
• Collaborates with physicians and practice staff to establish and implement care management plans based on evidence-based guidelines
• Triages patient calls, provides consultations on symptom management, and coordinates treatment plans across the institution

Required qualifications

• Bachelor's Degree in Nursing (Required)
• Active PA RN license (Required)
• Dual (NJ & PA) RN licensure required within 3 months of hire (Required)
• 1+ years prior nursing experience in internal/family medicine and care management experience (Preferred)
• BLS through AHA (Required)
